In the Middle School, the policy that Ed Tech has used for image attribution is presented below. This is what we have been using since I came to ISB in 2010. Some slight variation of this format may have been used by some teachers as well.
Students must understand that any image that they find on the web can't just go into their project without proper attribution.
What are their choices? Use Creative Commons (CC) images.
Why these? Internet users who upload their own images, sounds and text are increasingly using the Creative Commons license. A Creative Commons license allows a publisher to indicate how and under what conditions someone else can make use of their content.
Where can they find CC images? Below are two image search engines tailored to efficiently locate images for blogs, inspiration, research, etc.:
How do they cite the image? When using an image on a website don't forget to put the link to the original image. The citation could follow this format:
By publisher name, "photo title"(URL) via source, Creative Commons License

By danielito311, "Ceviche" via Flickr, Creative Commons Attribution, Non commercial
By blmiers2, "Trees" via Flickr, Creative Commons Attribution
